About the role:
You will report to the team or practice manager. You will need to be able to manage a caseload that involves undertaking the full legal range of social care assessments and developing person-led support plans to meet the needs and outcomes of individuals. To be responsible for ensuring the delivery of high quality and legally compliant social care practice to agreed standards for the staff within the remit of the post. To maximise the independence and wellbeing of adults and older people. You will act in accordance with all policies and procedures which apply to the job and understand the reasons for this.
You will need to have an ability to respond appropriate on reflect in action using your sensitivity, persuasiveness, negotiation and assertiveness skills which are required to communicate with diverse audiences in emotive circumstances. You will need to have the ability to build trust and confidence with adults and older people, carers and colleagues. You may need to manage challenging behaviour and difficult situations such as complaints or not wanting to engage. You need to be able to maintain good working relationships with other professionals whilst challenging the decisions of other statutory agencies.

Key Responsibilities:
Completing annual reassessments / reviews for service users and informal carers.
Identifying safeguarding concerns and be able to complete the safeguarding process.
Help people to be aware of and access universal services, voluntary sector and their local support networks.
Keep records updated
Using Mosaic for recording the assessment
Make appropriate referrals to our partner agencies.
The current initiatives within in the team is focusing on a person’s strengths and how to enhance their informal support network. This work has been undertaken with the Adut Early Help Team.
There will be other projects and initiatives that will become available as the year progresses.
You will not have any supervisory responsibilities.
Supervision of social care assistants, student social workers as required.
Qualifications and Requirements:
To be considered for this position, you should meet the following requirements:
You need to be a Qualified Social Worker and registered with Social Work England
You need to be qualified for a minimum of 2 years.
You need to have good communication skills, be able to work as part of a team as well as using your own initiative when working on your own.
You need to have experience of using Mosaic, Teams, Microsoft, Outlook, Oracle
You need to be physically able to complete home visits in the community which may include stairs for access. There role will also include some out of borough visits.
