About Us: 

The review team are responsible for completing annual reviews which are due or overdue for service users and informal carers as well as the short pieces of work that come from those reviews. The social workers in the team complete safeguarding processes if they have identified safeguarding concerns. We complete visits in the community and at times there are some out of borough reviews.  As a practice manager in the team, you would be line managing up to 4 practitioners and act for the team manager in their absence. Manage a complex and varied caseload, including a safeguarding lead role and investigating complaints, within a framework of policies and procedures.

About the role: 

Conduct statutory re-assessments or reviews of adults & older people.  

Provide professional expert advice and guidance to colleagues and partner agencies in supporting adults and older people.

Maintain properly documented case files, records and information.   Prepare standard reports as required and complete monthly audits.

Mentor and support others in their development.

Carry out all duties with an awareness and understanding of the Safeguarding requirements within area of responsibility and provide professional support in ensuring the best outcomes for residents with assessed care and support needs. Making Safeguarding Personal. 

All opportunities to prevent, reduce and delay access to social care services are taken to maximise resident’s abilities to maintain their independence. 

To be responsible for ensuring the delivery of high quality and legally compliant social care practice to agreed standards for the staff within the remit of the post. The practice manager needs to be an expert in managing uncertainty, developing trusting and respectful relationships with people, making fine judgements about risk and daring to work creatively and innovatively. Act in accordance with all policies and procedures which apply to the job and understand the reasons for this.

Key Responsibilities: 

Allocate work, quality assure completed pieces of work and authorise so can be progressed

Monitor annual review folders and quality check  work being placed into the folders

Support the team with reflective  work discussion, giving direction if needed

Be able to act for the team manager in their absence

Actively participate in projects as they arise including high and low cost support plans

As part of the reassessment/review process ensure a strength based approach is used to ensure support plans reflect the level of need appropriately

Supervise a minimum of  4 members of the team, this may include supporting a student social worker or a newly qualified social worker

 

Qualifications and Requirements: 

To be considered for this position, you should meet the following requirements: 

Social worker qualification and registered with Social Work England

To have at least 3 years post qualifying experience and have experience of line management or supervising others

Able to use microsoft  word, spreadsheets and all local government recording systems relating to the post 

Physically able to complete home visits which may include stairs

 Practice Educator qualification
