About the Role

The Community Learning Disability Team is co located with colleagues in health . They are dedicated to assess , support plan and complete interventions for people aged 18 upward who have a diagnosed Learning Disability and connected needs (note that there is some work related to planning and preparation for adulthood aged 14-17) Main tasks are as follows

  • Assessing the care and support needs of adults/carers in accordance to guiding legislation The Human Rights Act (1998) / The Mental Capacity Act (2005) / The Autism Act (2009) / The Equality Act (2010) / The Care Act (2014) and The Down Syndrome Act (2022)
  • Interventions that relate to safeguarding adults
  • Completing capacity assessments and when required Best Interest Meetings/decisions in line with the Mental Capacity Act 2005.
  • Supporting adults who may lack capacity to consent to their accommodation and care needs and applying to the Court of Protection for authorisation of any deprivation of liberty.
  • Managing a caseload providing a social work service to people with care and support needs, which will include adults at risk of harm and neglect.
  • Undertaking administration (including keeping of appropriate up-to-date records) and management of personal workload providing statistical information as required.
  • Applications and oversight when a person’s presenting needs mean that they could be eligible for CHC (Continuing Health Care) Funding
  • The role will require evidenced registration with Social Work England.
