Hours Per Week: 36 hours- 4 working days, subject to rota pattern & 18 hours- 2 working days, subject to rota pattern

Flexible working

We are looking for an enthusiastic and motivated practitioner to join our newly established Early Support and Advice Hub (ESAH).

ESAH provides a response to new referrals into Adult Social Care gathering vital information to provide early help and advice.

ESAH also gathers all referrals for those with existing care and support arrangements in place and ensures they are referred over to long term support teams for a timely response.

ESAH has a key focus on maximising independence mitigating risks and maximising independence. Keeping people living in their own homes, independently for as long as possible. ESAH utilise key relationships to achieve this goal inclusive of the third and voluntary sector. The Team have a strengths-based approach utilising direct payments, assistive technology as well as reablement support.

The team provide a 7 day service; which covers from 7.30am to 7pm. The successful post holder will be part of a rota pattern to maintain service provision which includes weekends and bank holidays. This role is predominantly a desk based role with no home visits however, flexibility is required at times to meet the demands on ESAH team and therefore, home visit support may be requested at times.

Hybrid working is available within this role.

As an assessor within the team you will receive regular supervision, support and training to ensure your continuous professional and personal development.

About You

Experience of working with adults and previous employment in a similar position would be desirable.

It is essential that you hold a current valid driving licence and this post will require an enhanced DBS disclosure.
