The Role
It is an exciting time to join Tameside council as we begin our plans to transform and redesign our Reablement & Early Support Services.
Tameside Council is committed to supporting people to live independently, stay well, and access the right support at the right time. We are looking for an experienced and motivated Service Improvement Lead for Early Support to help us strengthen prevention, early intervention, and communitybased support within Adult Social Care.
The Service Improvement Lead for Early Support will be lead and deliver service improvement and transformation initiatives focused on prevention, early intervention, reablement, and strengthsbased practice. The postholder will be required to use performance data, insight, quality assurance, and lived experience to identify improvement opportunities and measure impact.
This is a key role within our improvement and transformation function, working across Adult Social Care, health, housing, and voluntary sector partners to reduce escalation into longterm care and improve outcomes for local people.
To support the senior management team in the formulation of the overall strategy for the Integrated Care and Support Service, setting policy and performance targets for the service, supporting the Transformation agenda and budget strategy.
The Strategic Service Improvement Lead will lead for the directorate on strategic business partnering across the Adult Social Care directorate, corporate departments of the council, health sector and other external stakeholders to ensure the sustainability of effective, value for money services and that people are placed at the heart of what we do.
The role will support the Service to deliver a focused programme of transformational projects that build on the significant pathway redesign completed last year. With demand increasing, referral complexity rising, and system pressures intensifying, this year’s priorities centre on strengthening clinical capacity, enhancing workforce capability, improving system flow, and embedding a more advanced rehabilitation model. Collectively, these projects will increase independence outcomes, reduce avoidable long term care, support timely hospital discharge, and ensure the service is resilient, clinically robust and future ready.
About You
We are looking for a dynamic and highly motivated person to lead on the delivery of transforming Early Support services which include our reablement team. These teams provide short interventions to maximise independent living to enable people to live well at home for as long as possible. The Teams provide statutory and non-statutory Assessment and reassessment of individuals.
You will be managed and supported in your role by the Service Unit Manager. You will need to be an effective team player who has experience of partnership working and networking with colleagues within Social Care and Health.
You will need to have excellent communication skills and the ability to work in partnership with the wider Social Care and Health system leaders to promote and bring about positive change through this period of transformational change.
You will need to effectively manage and prioritise your own work load and have the ability to work in a flexible and sensitive manner within a demanding service.
