About the Team
Our Reablement Service provides time‑limited support (typically up to six weeks), usually in people’s own homes, helping people work towards their reablement goals so they can live more independently and stay in their communities. We work as a therapy‑led, multi‑disciplinary team alongside Team Leaders, Senior Reablement Practitioners, Reablement Practitioners and Therapists. Hybrid working is supported, and our frontline services operate seven days a week, 365 days a year.
About the Role
You will provide advice, support, and direction to Reablement staff during evenings, weekends, and Bank Holidays, ensuring a safe, responsive, and high‑quality service. This includes managing staffing resources, redeploying workers where necessary to ensure all visits are covered and responding to situations where service users or staff may be at risk.
The role involves making timely decisions, coordinating risk‑management actions, and liaising with the Emergency Duty Team when required. You will also ensure that local Reablement teams are kept fully informed of actions and outcomes.
You will work closely with internal colleagues and external partners, including health, housing, and independent sector home care providers, to maintain continuity of care and support vulnerable individuals.
Your Application
In order to be considered for shortlisting, your application will clearly evidence the following skills and align with
our behaviours:
Experience working in reablement, adult social care, or a related health and social care setting
Ability to make sound decisions independently and recognise and respond appropriately in out‑of‑hours situations
Experience supporting or supervising staff, including allocating or redeploying resources
Demonstrable understanding of safeguarding, risk assessment, and lone‑working procedures
Strong communication skills, with the ability to liaise effectively with professionals, service users, and partner agencies
Ability to work under pressure and manage competing priorities
Experience using electronic care or case‑management systems with good record-keeping skills
