This full-time position has a starting salary of £63,999 per-annum based on a 36-hour working week. Part-time applications cannot be considered for this role. We have a great opportunity to join our St. Peter’s Hospital, Social Care team as an Adults Team Manager . This is a hybrid role, working from the office base at least three days per week. Our service operates from 8am to 8pm, Monday to Friday, and 8am to 4pm at the weekend. This Team Manager role covers Monday to Friday. The post is located in West 2 Area of Surrey County Council and will be in West Surrey Unitary Authority from 1st April 2026. You will be a key partner supporting the delivery of the operational innovation and strategic ambitions for the Transfer of Care Hub (TOCH) and the Hospital Discharge to Assess, Home First Approach. Your role will involve managing discharge activity from Ashford & St Peters Hospital NHS Foundation Trust, including timely transitions from the Discharge to Assess pathway within the community, while maintaining strong, collaborative relationships with health partners, professionals and wider stakeholders. You will communicate effectively and lead within your team and as part of the West 2 Area, Leadership team, organisational change, including the Local Government Reorganisation (L.G.R.) supporting the team to continue meeting service demands during periods of transition. You will be accountable for approximately a team of 20 people including Social Workers and Social Care Practitioners, Assistant Team Managers and Seniors roles within both professionally and professionally unqualified roles with overall responsibility for team performance. The team plays a vital role in delivering Surrey County Council’s, Adult Social Care vision, particularly the home-first approach, strengths-based practice and the ambition to leave no resident behind. As the Team Manager you will have a significant impact on hospital flow and positive discharge outcomes, supporting NHS partners to provide the right care at the right time, working proportionately, whilst ensuring Care Act 2014 compliance, while also ensuring services are delivered in a cost-effective way through the best use of internal resources and voluntary, faith and community sector provision. As one of five acute hospital teams within Surrey, Ashford & St Peters Hospital supports approximately 30-35% of the total population of Surrey, the service supports residents and families to access the right short-term support, care packages or placements on discharge, alongside community partners, enabling assessment and recovery in the community and promoting people to regain independence as quickly as possible. Your Application In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours : professional Social Work or Occupational Therapy qualification with current registration to the relevant professional body Significant post-qualification experience in adult social care, including management experience and a strong understanding of social care and health agendas Sound working knowledge of Adult Social Care legislation, including the Mental Capacity Act, NHS Continuing Healthcare National Framework and safeguarding adults procedures Excellent communication skills, with the ability to manage and lead teams, provide effective professional supervision and build strong working relationships A proven track record of effective inter-agency and partnership working A strong commitment to strengths-based practice, promoting positive outcomes, community inclusion, and personal choice and control Surrey has both urban and rural areas and social care workers will be expected to have a valid driving licence to drive in the UK, access to a vehicle and be willing to travel across a wide geographical area.
