This full-time, permanent role has a starting salary of £42,239 per annum, based on a 36-hour working week. We have an exciting opportunity to join our East Surrey Hospital team as a full time Social Worker. This service runs from 8am to 8pm Monday to Friday and 9am to 5pm Saturday and Sunday. The role will require being based onsite with some hybrid working supported. Further information regarding the working pattern can be discussed at interview. Additional weekend shifts will be available to take on if desired. We want to ensure that ‘no one is left behind’ and that we can provide excellent customer service to the residents of Surrey, their families and close networks. Our team has recently undergone a restructure, with a new 'Home First and Transfer of Care Hub' (TOCH) now operating to work alongside our social care team. The TOCH is a joint health and social care initiative led by a social care manager, and we work together in a closely coordinated way. We are looking for the right person to continue in the development of this new system and ways of working. Taking on the role is a fantastic opportunity to develop your skills through promoting strengths-based and person-centred practice that is legally compliant; supporting our residents to be discharged from a large acute hospital setting in a safe and timely manner. As part of a changing team you will be able to adapt to changes and support new systems that help to improve the flow of people safely from hospital. Your key responsibilities as a Social Worker in our East Surrey Hospital team will include: Care Act assessments Mental Capacity Act assessments and leading on Best Interest Decisions Screening residents for Continuing Healthcare Carers assessments Safeguarding work and partnership working with our Hospital NHS colleagues, including Transfer of Care Hub and Discharge 2 Assess Attending multi-disciplinary team meetings and working with ward teams Supporting adults, advocates and families through the discharge process You will also work closely with Adult Social Care area and specialist teams as well as community health, charity and voluntary organisations Your Application In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours : Professional qualification in Social Work and current registration with Social Work England and completion of an Assessed and Supported Year in Employment if more recently qualified Knowledge and experience of current legislation, including the Care Act and the Mental Capacity Act and its application to practice in a social care setting. Knowledge of relevant guidance and person-centred strengths-based practice Knowledge of and experience in carrying out safeguarding enquiries, carrying out assessments within the hospital and/ or community and working with a diverse group of individuals and partner agencies Experience of supporting non-qualified or less experienced members of staff with their development Commitment to your own professional development An enhanced DBS ‘Disclosure and Barring Service’ check for regulated activity (formerly known as CRB) and the Children’s and Adults’ Barred List checks will be required for this role.
