Your Role

Due to extended funding, there is an exciting opportunity for an experienced Social Worker to work under a three-year programme to deliver on the Making Every Adult Matter approach within the Front Door Team of Adult Social Care in Stockport.

The role will consist of completing Care Act assessments with people who are homeless, in temporary accommodation or at risk of eviction, involving individuals who are considered to have multiple areas of disadvantage.

Also working with partner agencies to support vulnerable and at-risk Adults, ensuring the most appropriate support is offered and in place.

About You

You will be a qualified Social Worker who has completed an ASYE program and be an experienced worker who is working towards progressive practice education or achieved this already. You will have extensive knowledge and skill and a keen interest in supporting Adults who may have involvement with substances, mental health, homelessness and complexity in their lives.

The Front Door team are a multidisciplined team with extensive expertise in ensuring the Residents of Stockport have access to the right support at the right time, with an emphasis on Prevention, Wellbeing and Independence. The Making Every Adult Matter programme is a multi-agency initiative which aims to support the most complex Residents of Stockport in a multi- agency and strengths-based way.
