We have an exciting opportunity for a part-time Experienced Social Worker who is passionate about making a positive difference to the lives of young adults, in our Preparation for Adulthood team (part of Adult Social Care). 

You will be responsible for assessing needs and managing support arrangements for a range of young people, helping bridge the gap for them and their carers between children’s and adult social care services. You will be responsible for arranging support for young people between the age of 18 – 25. However, you will also support and prepare young people and their families for the move from children’s to adult social care before those young people become 18.

Your responsibilities will include undertaking assessments of need, creative support planning and making safeguarding adults enquiries. Successful candidates will be working in partnership with young people, their carers, colleagues in children’s services, education settings, specialist health services and with a range of partner organisations.

You will need to be a strong advocate of enablement and strengths-based practice. You will have the drive to learn new skills and to keep your knowledge up-to-date and share it with colleagues.

Please note; this role is a temporary post for 10 months (from the start of you taking up the post) to cover a secondment in the team and is for 21 hours per week.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
