Social Care Practitioner
Sheffield City Council
Are you passionate about making a positive difference to people’s lives? Sheffield City Council is looking for motivated and compassionate Social Care Practitioners to join our Adult Social Care service and help us deliver high-quality, person-centred support to adults across the city.
This is an exciting time to join us. We are strengthening and developing our services so that people receive the right support, at the right time, in a way that promotes independence, choice and wellbeing. We are looking for people who are enthusiastic about working creatively, embracing change and contributing to a modern, strengths-based adult social care model.
Our Home First – First Contact Team provides a welcoming and responsive first point of contact for adults, families, carers and professionals seeking adult social care support in Sheffield.
The team offers a wide range of support, including information and advice, signposting to community services, preventative interventions, and the completion of person-centred assessments and support plans. You will work in a supportive and committed team that values collaboration, professional curiosity and respectful practice. We are looking for practitioners who can build positive relationships, recognise people’s strengths and networks, and support safe, proportionate and effective decision-making.

We are seeking applicants with recent experience in adult social care or a related field, good IT skills, and an understanding of key legislation and practice areas including the Care Act 2014, the Mental Capacity Act 2005 and safeguarding. Strong communication skills, excellent customer care and a commitment to collaborative working are essential.
You will receive a positive induction, ongoing support and opportunities to develop your skills and confidence within the role. We also offer flexible working arrangements that support a healthy work-life balance and help our staff to thrive both inside and outside work.
