Maximising Independence Team - Experienced Social Worker
A new exciting opportunity for an experienced social worker to join our innovative team has arisen.
Would you like to work as part of a new multi-disciplinary team, working with people who have complex and enduring mental health challenges, who are supported by adult social care?
The maximising independence team will help people to become more independent by reassessing people currently supported in the Adult Mental Health Social Care Service , and supporting them move on from long-term provision via an MDT approach to (re)gaining abilities and skills, set enablement goals, and take advantage of opportunities to grow and develop their lives.
Your role will be to work alongside other social workers, social care practitioners, occupational therapy and support workers to deliver key activities with targeted groups of people including: young people moving form children's services into adulthood, people in residential placements who could take advantage of supported living opportunities, and supporting people in supported living settings to move to private tenancies with the right care and support arrangements. You will need to have a good understanding working with the Care Act 2014 and the Mental Capacity Act 2005, and the Mental Health Act.
Knowledge, skills and experience of supporting people with complex mental health challenges to move on from intensive service delivery to less supported settings would be desirable in this role.

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.
If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
