About Us

The Adult Social Care Contact Team is often the first point of contact for people seeking support, advice, or information about adult social care in Salford. We work in a fast-paced, strengths-based environment, helping people identify solutions that maximise independence, wellbeing, and community connections. We have strong links with voluntary, community, social enterprise (VCSE) organisations, health services, partner agencies, and local communities, ensuring people can access the right support at the right time. Through early help and prevention, we aim to promote positive outcomes and reduce the need for longer-term intervention wherever possible.

About the Role

This is an exciting opportunity to join our Adult Social Care Contact Team as a Community Assessment Officer. You will undertake strengths-based conversations, complete early help and initial assessments, carry out community visits where required, and work with people to identify solutions that build on their existing strengths, networks, and community resources. You will have a key role in connecting people to local support through our wider early help and VCSE offer, whilst working closely with health professionals, social workers, housing providers, and partner agencies. No two days are the same, and you will be at the heart of helping people access the right support at the earliest opportunity.

About You

We are looking for a motivated and compassionate individual with excellent communication, assessment, and organisational skills. You will be passionate about strengths-based practice and committed to supporting people to maintain their independence, wellbeing, and choice. You will be confident gathering and analysing information, undertaking community visits, managing competing demands, and working collaboratively with colleagues and partners. Experience within health, social care, customer contact, or community-based services is desirable, alongside an understanding of early intervention, professional curiosity, and person-centred practice. You will share our commitment to helping people find solutions that build on what matters most to them.

Please be aware that this post is subject to a Disclosure and Barring Service (DBS) check and that you will be required to meet the cost of the DBS check currently £49.50. The payment for this will be deducted from your first four months’ salary payments at a rate of £12.38 per month. If you do not commence employment for any reason but the DBS has been processed, you will be sent an invoice for the payment of £49.50.
