About the role

Rochdale Adult Care is pleased to offer an exciting opportunity for a permanent Assessment and Support Planning Officer within the South Integrated Neighbourhood Team (INT).

The role involves working with adults, their families and carers to identify strengths, assess needs and support people to achieve the outcomes that matter most to them. You will complete strengths-based assessments, reviews and support planning, promoting independence, wellbeing and choice in line with the Care Act 2014.

We welcome applications from individuals with experience in Adult Social Care and those with transferable skills from related sectors, who can demonstrate a commitment to person-centred practice and achieving positive outcomes for adults with care and support needs.

Working within a busy multidisciplinary neighbourhood team, you will work collaboratively with colleagues, health partners, providers and community organisations to support adults to remain safe, independent and connected within their local communities.

About you

We are looking for a motivated and organised individual with excellent communication and written skills, who can build positive relationships with adults, families and professionals.

You will be able to demonstrate:

  • Experience of working with adults, families, carers or vulnerable people.
  • Strong assessment, problem-solving and organisational skills.
  • The ability to manage a varied workload and work to timescales.
  • A person-centred and strengths-based approach.
  • Confidence in working with a range of professionals and agencies.
  • Good IT and recording skills.

Experience within Adult Social Care is desirable; however, applications are welcomed from candidates with relevant transferable skills and experience from health, housing, voluntary, support, community or related services.

A willingness to travel across the borough in line with service needs is essential.
