About the role

Rochdale Adult Care is pleased to offer two 6-month temporary opportunities for Assessment and Support Planners within the Prevention team, offering an excellent opportunity to join a vibrant and creative authority.

The post holder will be expected to be a dynamic fast paced individual, who holds the appropriate skills, experience and motivation to work with Adults and their carers, whilst working within timescales and deadlines to produce a high quality work.

In this exciting role, you will have the opportunity to be part of prevention team for all new contacts coming into adult care, you will focus on delaying and reducing care and support needs by signposting, assessing and utilising assistive technology.

About you

The post holder will be expected to be dynamic fast paced individuals, who have excellent written and communication skills and a willingness to learn. It is expected that the post holders will have some knowledge or experience of working within an Adult Care setting, strength based approaches to assessments and support planning and supporting individuals who draw on care and support, promoting independence and well-being, as within the objectives of the Care Act 2014.

Hours are required to be worked 5 days over 7, as per the needs of the service.

A certain amount of travelling will be required, as per the needs of the service.
