Rochdale Adult Care is pleased to offer permanent opportunity for Triage Prevention Assistant within the Prevention team, offering an excellent opportunity to join a vibrant and creative authority.

The role will have particular emphasis on assistive technology, careline and carers. The post holder will complete assistive technology referrals/ assessments and fitting and undertake carers assessments and support plans.

The post holder will be expected to be a dynamic fast paced individual, who holds the appropriate skills, experience and motivation to work with Adults and their carers, whilst working within timescales and deadlines to produce a high quality work.

In this exciting role, you will have the opportunity to be part of prevention team for all new contacts coming into adult care, you will focus on delaying and reducing care and support needs by signposting, assessing and utilising assistive technology.

About you

The post holder will be expected to be dynamic fast paced individuals, who have excellent written and communication skills and a willingness to learn. It is expected that the post holders will have some knowledge or experience of working within an Adult Care setting, strength based approach to supporting individuals who draw on care and support, promoting independence and well-being, as within the objectives of the Care Act 2014.

The post holder will be expected to undertake a certain amount of travelling in the course of his/her duties for which a casual car user allowance will be paid.
