Role Overview:

The Oldham Health and Social Care economy has signed up to a new and ambitious vision for care which looks to ‘see the greatest and fastest possible improvement in the health and wellbeing of the Borough’s residents. To support the vision we have brought together a range of partners including for example Adult Social Care, Pennine Care NHS Foundation Trust, Northern Care Alliance, MioCare, primary care and the Oldham Integrated Commissioning Board to create a new integrated system approach Oldham Cares.

As part of this is vision we are looking for a vibrant experienced social worker to join our integrated team. We are looking for a passionate, experienced enthusiastic Social Worker to support patients to be discharged from hospital under the D2A process.

You will be qualified to degree level and be SW England registered.

Your main duties will be to provide an assessment, support planning and review service, underpinned by professional knowledge and legislation in order to ensure that:

  • Complex individuals are supported appropriately including undertaking mental capacity assessments and best interest processes
  • Vulnerable adults are safeguarded and are supported to live well with risk;
  • Strength based conversations that result in person centred outcomes and support requirements are identified, and;
  • People are encouraged and enabled to live healthy and independent lives.
  • Advocate for residents to ensure their voices are heard.
  • Discharge plans are co produced with our residents and their support systems

Key Responsibilities:

  • To undertake the assessment of support needs for Service Users, Carers and Families, where their circumstances may be difficult and unstable, and assist in screening referrals.
  • To be proactive with respect to Safeguarding issues and following relevant policies and procedures if abuse is suspected.
  • To identify, assess and manage risks to Service Users, Carers and Families, and contribute towards a positive change to behaviour to reduce risk.
  • To respond to crisis situations, making assessments about urgency and using appropriate legal intervention.
  • To ensure all relevant systems are updated in a timely manner and information/outcomes are accurately recorded. To maintain a high standard of data quality on social care systems. To ensure all personal information is confidential.
  • To prepare, attend and participate in meetings and decision-making forums, for example, Court appearances, Reviews, Panels, Case Conferences or team meetings. To chair meetings as appropriate.
  • To act for and advocate on behalf of Service Users, Carers and Families, within the boundaries of corporate and directorate policies.


Key Requirements:

  • Experience of implementing, monitoring, evaluating and updating Care & Support Plans through person centred planning principles and relevant procedures
  • Ability to negotiate with others both internal and external, e.g. health service or voluntary sector organisations, to secure appropriate services for Service Users, Carers and Families
  • Detailed knowledge of the relevant legislation, codes of practice, National Guidelines and Government initiatives connected with service delivery in this field of social work
  • Knowledge and understanding of equalities and diversity issues in connection with delivering social work
