Community Reablement Team Leader

Newcastle City Council is looking for an experienced, motivated and compassionate Community Reablement Team Leader to join our Community Reablement Service.
This is an exciting opportunity to lead and support a team dedicated to helping people regain independence, maximise their abilities and achieve the outcomes that matter most to them.
As a Team Leader, you will play a key role in supporting adults to remain in their own homes, preventing unnecessary hospital admissions, facilitating safe hospital discharge and reducing long-term reliance on care and support services.
About the Team
Community Reablement is a dynamic, city-wide service supporting adults following illness, injury, hospital admission or changes in their circumstances. Our aim is to promote independence, build confidence and enable people to do as much as possible for themselves.
Working closely with health colleagues, social care professionals, families and carers, we provide short-term, goal-focused support that helps people regain skills and remain safely in their own homes.
The service operates across Newcastle and is committed to delivering high-quality, person-centred support that aligns with the Care Act, Care Quality Commission (CQC) Fundamental Standards and the principles of strengths-based practice.
About the Role
This is an excellent opportunity to provide leadership within a fast-paced and rewarding operational service. You will support the Resource Manager in the day-to-day management of Community Reablement Services, providing supervision, guidance and operational oversight to a team of Health and Social Care Officers, Coordinators and other staff. You will be responsible for ensuring services are delivered safely, effectively and in accordance with regulatory requirements.
The successful applicant will be a confident and inspirational leader with experience of working in adult social care, health or reablement services. The applicant will be passionate about helping people achieve independence and positive outcomes, be an effective people manager with experience of leading and motivating teams. They will be committed to continuous improvement and delivering high-quality services, skilled at managing competing priorities in a busy operational environment and knowledgeable about safeguarding, risk management, regulatory requirements and quality standards.
Most importantly, you will share our commitment to supporting adults to live independently and to delivering compassionate, person-centred care and support.
