An exciting opportunity has arisen for a Social Care Assessment Officer to join Adult Social CarePoint. Adult Social CarePoint is the first point of contact for adult social care enquiries at Newcastle City Council. 

You will work alongside a multi-disciplinary team of professionals to effectively promote independence, wellbeing and choice for adults and carers across Newcastle. The team is committed to person-centred practice, building upon the adults strengths to support them to live safely and independently.

This is an exciting time to join Adult Social Care in Newcastle as we continue to develop our multi-disciplinary approach ensuring people receive an effective and responsive service. 

Your responsibilities will include working in close collaboration with our multi disciplinary team, completing Care Act assessments, identifying whether a person is eligible for support using a strengths based approach, completing Carers Assessments, and be guided by social care legislation.

You will visit people at home, provide information, advice and commission appropriate care and support to enable the person to live safely and independently. You will liaise with professionals, agencies and service providers to ensure the right support is provided at the right time. You will review care packages to ensure appropriate support is in place. The role involves commitment, flexibility and the ability to make quick, sound judgements.

You should have experience of working within adult social care and be willing to undertake further training to enhance your skills.

Supervision and an annual appraisal is provided by the Team Managers and Senior Practitioner within Adult Social CarePoint.

You will mainly be office based with some opportunity for hybrid working. You will also be carrying out regular home visits to adults living across Newcastle.
