The London Borough of Merton's Targeted Review Team offers an opportunity to apply your professional skills in a role focused on reviewing, evaluating and improving support arrangements for adults receiving services across Adult Social Care.

About the Role
As a Social Worker within the Targeted Review Team, you will work across Adult Social Care services undertaking reviews of individuals receiving support. Your work will help ensure services remain person-centred, proportionate and aligned with assessed needs, while promoting independence and positive outcomes for residents and carers.
Using your professional knowledge and analytical skills, you will:
- Complete strengths-based reviews and reassessments.
- Evaluate support arrangements against assessed needs and outcomes.
- Make evidence-informed recommendations regarding care and support provision.
- Work collaboratively with residents, carers, providers and multidisciplinary colleagues.
- Support effective decision-making and resource allocation.
- Ensure care and support plans accurately reflect needs, strengths and goals.
- Contribute to quality assurance and continuous service improvement.
