A bit about us:

Liverpool is home to vibrant, energetic, and engaged communities, people who take real ownership of their local area and are passionate about making our city a better place. We've got ambitious targets to meet and we need people who are driven, dynamic and ready to deliver the very best for our communities. Public service is more important than ever, and this role represents a real opportunity to make a difference to our communities by transforming the way we work and placing our residents at the heart of everything we do.

Working at Liverpool City Council provides an opportunity to contribute to your community and make a positive impact on the lives of residents. Our values shape how we work to deliver public services, and how we communicate with the residents we serve. We care about the best outcomes for local people, through our public services.

We are proud of our world-famous thriving city with its rich culture, as well as affordable housing, good schools, beautiful green spaces – there’s really something for everyone!

The role:

This is an exciting opportunity to join Liverpool City Council's Preparing for Adulthood Team, a specialist city-wide service supporting young people with disabilities, autism, learning disabilities, physical disabilities and complex needs as they transition from Children's Services into Adult Social Care.

As a Social Worker within the Preparing for Adulthood Team, you will play a key role in supporting young people and their families at a significant stage in their lives. Working alongside colleagues in Education, Health, Children's Services and Adult Social Care, you will help young people plan for their future and achieve the best possible outcomes as they move towards adulthood.

The team works in a strengths-based and person-centred way, building on the skills, abilities and aspirations of young people, their families and wider support networks. Using the principles of the Care Act 2014, you will promote independence, wellbeing and choice through effective assessment, support planning and review.

As a Social Worker, you will:

  • Complete strengths-based Care Act assessments and outcome-focused support plans
  • Work creatively with young people, families and partner agencies to identify solutions that maximise independence and wellbeing
  • Undertake Mental Capacity Assessments, Best Interest assessments, risk assessments and reviews where required
  • Support young people through complex transitions, ensuring that services are coordinated and planned effectively
  • Hold and manage a varied caseload, making informed and evidence-based decisions in line with legislation, policy and professional standards
  • Lead and contribute to multi-disciplinary meetings with Education, Health and Social Care professionals
  • Work in partnership with carers and complete Carers Assessments where appropriate
  • Apply the Care Act principles of Prevent, Reduce and Delay throughout all areas of practice
  • Commission and review support services to ensure individuals achieve their identified outcomes and make best use of available resources.

Preparing for Adulthood is a rewarding and fast-paced area of social work. We are looking for practitioners who are passionate about empowering young people, skilled in strengths-based practice and committed to achieving positive outcomes through collaborative working.

You will join a supportive and experienced team that values professional development, reflective practice and innovation. Whether you are an experienced practitioner or recently qualified and keen to develop your career within Adult Social Care, we can offer excellent opportunities for learning, support and progression.

Our offer to you:

  • At least 27 days holiday, plus bank holidays rising to at least 32 after 5 years’ service (pro rata if part time) - option to purchase further days
  • Agile working (role dependent) - a “Flexibility First” approach to agile working, where flexibility is the norm rather than the exception
  • Part of the Local Government Pension Scheme – option to join the AVC Scheme
  • Progression - opportunities and training through a wide range of high quality learning
  • Health and Wellbeing - offer including Employee Assistance Programme
  • Cycle to Work and Travel Pass Schemes
  • Drive Time – a salary sacrifice car lease scheme
