As a Wellbeing Worker you’ll thrive on working within an innovative and transformative Adult Social Care service. Working in the community with adults who have additional support needs.

- Good judgement when assessing risk, recording information accurately and following policies and procedures
About the role
As our next Wellbeing Worker, you’ll play an important role in supporting adults with care and support needs to live well, maintain their independence and achieve better outcomes. Working within Adult Social Care, you will complete person-centred assessments, identify strengths and support needs, and help people access the right combination of community resources, informal networks and care services.
This is a varied and rewarding role where you will work directly with service users, families, carers and partner agencies to develop practical support plans that reflect assessed needs, promote wellbeing and make effective use of available resources.
The role is integral to our Access and Care Delivery service, where you will work as part of a supportive team under the direction of a Team Manager, with guidance from Social Work colleagues. You will manage an appropriately allocated caseload, keep accurate records, contribute to reviews and escalate any safeguarding or practice concerns as needed.

Job Description
Job purpose
Under the direction of a Team Manager and with support and direction from Social Worker colleagues, provide a high quality and comprehensive assessment and support planning service, advising adults with care and support needs. In doing so, promote the principles of wellbeing as described within the Care Act 2014.
The post holder must at all times carry out responsibilities within the City Council’s Policies and Procedures, in particular the Council Policies on Equality and Diversity, Health and Safety.
Responsibilities
- Undertake appropriately allocated non-complex assessments and support planning deemed suitable for allocation to Wellbeing and Recovery Worker. Self-directed support options should be at the heart of all support planning with service users.
- With supervision, develop and maintain effective relationships with service users, their families and carers: provide advice and support: help resolve conflict,
- With support and supervision, contribute to the enhanced screening of new referrals and people already in receipt of services.
- With supervision, work with individuals, families, carers and communities to help them make informed decisions enabling them to clarify and express their needs and contribute to service planning.
- With support from Social Workers and supervision from the Team Manager, to manage an appropriately allocated caseload of non-complex work, regularly monitor, review and evaluate changes in service user needs and ensure that all services contribute effectively to the support plan and remain an effective use of resources.
- Where safeguarding issues arise, to escalate them to the manager and work with the Social Worker to support, contributing to protecting the service users.
- Contribute to the development of the service by actively engaging in workshops, consultations and engagement events.
Qualifications
- QCF Level 4 diploma in Adult Care would be desirable but not essential

- Able to effectively plan and engage service users in developing their support plan using a range of agencies.
- Ability to plan, prioritise and time manage own workload.
- Able to communicate well and with a range of partners and service users, carers and colleagues
- Able to engage individuals, families, carers and workers from other agencies
- Ability to devise and implement service and support plans
- Ability to assess the needs of and work with individuals and groups for people using a person centred approach
- Ability to maintain accurate service user records using IT and follow policy and procedure
- Able to exercise individual judgement, assess risk and consult where and when appropriate
- Report writing and Communications Skills
Knowledge required
- Of relevant legislation appropriate to the needs of service users from a wide range of backgrounds
- Awareness of the broad range of factors that impact on service users and families e.g. drugs, alcohol or domestic violence
- Understanding of the roles and responsibilities of statutory and voluntary agencies.
- Basic awareness of IT systems including Microsoft Office, Internet and Email
- Demonstrate an understanding of the importance of confidentiality.
- Demonstrate an awareness of needs and safeguarding issues associated with vulnerable people.
- Demonstrate an understanding of diversity issues within local communities.
