About you
As a SkILs Support Worker, you will bring to the role:
- awareness of the needs of older people and people with disabilities from diverse backgrounds, and an empathetic and enabling approach
- good communication skills and collaborative approach to work with various stakeholders
- a person-centred approach, with the ability to respond to changes in customers’ skills, confidence and support needs
- physical ability to undertake moving and handling tasks
- flexibility to travel around the city, with a valid driving licence and use of a vehicle (essential car user allowance payable)
We will support you to study for a Level 2 Adult Care Worker Apprenticeship within 12 months of starting in post.
About the role
As a Reablement Support Worker, you’ll support individuals to regain their independence following a change in circumstances,
illness, injury, or a hospital stay.
It’s not just about helping with everyday care and activities; it’s about enabling people to rediscover their abilities and maximise their potential.
You’ll be a key member of our Active Recovery reablement team. The team delivers short-term support in an individual’s own home that helps them to build the confidence and skills they need to live independently day to day and improve their quality of life.
You will:
- support customers with personal care and daily routines, with a focus on enabling them to achieve independence in these activities
- contribute to reablement plans, reviews and risk assessments and follow the recommendations when delivering care and support
- work closely with the customer, their family, carers and other professionals involved in their support
- enable customers to access local amenities and be part of the community beyond their home
- promote dignity and wellbeing, respect confidentiality and safeguard adults from abuse, in line with council procedures and policies
Skills, experience and qualifications
We provide full training and will support you to achieve relevant qualifications. You will complete the Care Certificate within 12 weeks of starting in post
and undertake a Level 2 Adult Care Worker Apprenticeship within 12 months of starting in post.

Job Description
Job Purpose Work within the Active Recovery service to provide short-term, person-centred reablement support, enabling adults to regain independence following illness, injury, or a change in circumstances, in accordance with agreed recovery support plans that promote choice, control, and social inclusion.
Responsibilities
- Provide support with personal care utilising strengths based person-centred approaches to enable people to maximise their independence and recovery.
- Support people to access local amenities and community outside of their home as part of a reablement plan, supporting people with daily living activities as identified in their support plan and as part of a package of support including personal care.
- Support with medication in accordance with agreed support plans, maintaining accurate and up-to-date records at all times
- Work as part of the Active Recovery multidisciplinary team including Occupational Therapists and Physiotherapists.
- Contribute to and utilise recovery support plans and person-centred risk assessments to promote independence in a safe environment.
- Undertake moving and handling including use of equipment.
- Support people in the use of community equipment, telecare, and adaptations.
- Contribute to reviews considering a person’s ongoing support needs.
- Promote dignity and wellbeing, and safeguard adults from abuse and ensure all safeguarding concerns are reported to a line manager.
- Respect confidential nature of work ensuring customers and carers are aware of Councils policies relating to person’s access to files and to work within the appropriate information sharing agreements.
- Communicate with the person, their family, carers, and other professionals involved in their support, as necessary.
- Accurately complete all necessary documentation, records, and reports as necessary and determined by the LCC procedures.
- Work flexibly and as part of a team across the city in accordance with the Council’s flexibility protocol.
- Deliver a high-quality service in line with CQC regulations and requirements.
- Assist with basic training and induction of colleagues and new starters.
- Work collaboratively with colleagues and health partners (e.g. Nurses, Occupational Therapists and Physiotherapists), knowing when to seek help and advice.
- Actively respect and consider all cultural, religious, personal, and social needs.

Qualifications Successful completion of the Care Certificate within 12 weeks of commencement and undertaking a Level 2 Adult Care Worker Apprenticeship within 12 months of starting in post are essential. Applicants must have a valid driving licence and access to their own vehicle for business use.
