This post is based in the East Kent area.
As a Case Officer within the Short-Term Pathways Team, you will be working with people over the age of 18 with a wide range of needs who have recently had a hospital admission. You will manage a small caseload requiring assessment, care and support planning and arrangement of on-going support to make a real difference in achieving improved outcomes and promoting independence. The Short Term Pathways team works collaboratively with health colleagues and other partnership agencies and focuses on promoting a person’s independence.
Through excellent opportunities for training and development, including a comprehensive induction, you will receive the support you need and recognition for your good work. The work related to this post includes the assessment of individuals across East Kent and a work base will be discussed at interview.
This role is subject to holding a Full UK Driving Licence – The Council is committed to making reasonable adjustments so whilst this job requires the jobholder to drive your application will still be considered if you are unable to drive due to a disability.
There is an expectation that the successful applicant will travel to various locations in East Kent to undertake Short-Term Pathways work. The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential.
This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements. The appropriate standards are set out in the Job Description/Person Specification.
The post is subject to an Enhanced Disclosure application to the Disclosure and Barring service.
