About The Role
This is an exciting opportunity to be part of our Community Information and Advice Team as a Carers Adviser.
In this role, you’ll be primarily working with unpaid carers across Hounslow, providing information, advice and practical support to help them access the right services and reduce the risk of carer breakdown.
You’ll work closely with carers through face to face outreach sessions, events, telephone and digital channels, offering guidance on Adult Social Care, carers’ rights, assessments, benefits and local services. The role involves managing a caseload, building trusted relationships with carers who may have more complex needs, and working in partnership with internal teams, commissioned services and voluntary organisations to ensure timely and appropriate support is in place.
Development opportunities include building expertise in carers’ legislation and entitlements, developing outreach and engagement skills, contributing to service improvement projects and borough wide campaigns (such as Carers Week), and gaining experience working across Adult Social Care systems and partnerships.
About The Team You’ll Be Working In
The successful candidate will join the Community Information and Advice Team within Adult Social Care, whose key aim is to ensure residents can easily access clear, timely and reliable information and support to help them live well and remain independent.
The team plays a vital preventative role, supporting carers and residents before issues escalate, helping to reduce crisis situations and improve outcomes for individuals and families across Hounslow.
The team brings together roles such as the Community Information Manager, Carers Advisers and other information and engagement officers, working closely with social care colleagues, commissioned providers and the voluntary sector.
The postholder will report directly to the Community Information Manager and work collaboratively with fellow Carers Advisers to deliver outreach, advice services and continuous service improvement.
About You
This role would suit someone who:
• Is passionate about supporting unpaid carers and understands the challenges they face
• Can communicate clearly and professionally with people from a wide range of backgrounds, both verbally and in writing
• Is organised and confident managing a varied caseload and competing priorities
• Is creative and proactive in planning ideas, outreach activities and events that engage carers and meet local needs
• Is comfortable working across different settings, including community venues, events and occasional home visits
• Is confident using IT systems, including caseload management tools and Microsoft Office, and values accurate record keeping

• You have a working knowledge of using IT, caseload management systems and using Microsoft Office, particularly Excel.
• You have a passion for wanting to support informal/unpaid carers and are willing to stay up-to-date with an ever-changing range of services, processes and legislation as well as working to find satisfactory solutions.
Additional Requirements:
Weekend and evening work will be required as part of this role
