Care Manager – Assertive Outreach (Severe & Multiple Disadvantage)

We are recruiting a Care Manager to join a new Adult Social Care–led Assertive Outreach Team, working with adults who are rough sleeping or at imminent risk of rough sleeping, many of whom face multiple disadvantages and significant barriers to engaging with statutory services.
This role focuses on practical engagement, assessment support, care coordination and prevention, working alongside a Social Worker within a small, specialist team embedded in the MHLDA Front Door.
You will play a key role in building trusted relationships, supporting assessment processes, coordinating care and working alongside partners to prevent the escalation of need and crisis.
Key Responsibilities
- Engage adults experiencing rough sleeping through assertive, relationship‑based outreach.
- Support Care Act assessments by gathering information, building engagement, and coordinating inputs.
- Assist with support planning, service coordination and practical problem‑solving.
- Work closely with Housing, homelessness services, health partners and voluntary sector organisations.
- Support safeguarding processes, including protective planning and follow‑up actions.
- Contribute to early intervention and prevention, reducing repeated A&E attendance and crisis responses.
- Support accurate, timely and person‑centred recording.
- Contribute to system learning and joint working with voluntary sector colleagues.
About You
You will:
- Have experience working in Adult Social Care, mental health, homelessness, substance misuse or related fields.
- Be confident working with adults facing multiple disadvantages and complex needs.
- Have a good understanding of the Care Act, safeguarding, and person‑centred practice.
- Be skilled in engagement, relationship‑building and working creatively to overcome barriers.
- Be organised, resilient and comfortable working flexibly in community settings.
- Bring a trauma‑informed and strengths‑based approach.
