The Transformation team is a dynamic and creative team that has a unique role in influencing and transforming services to ensure the department meets its vision and to champion the department’s behaviours. We use inclusive project management approaches and work in partnership with residents, staff and partners to deliver an ambitious programme of change. 

In this recruitment process, we are looking for candidates who: 

– Have a good understanding of adult social care, and a proven track record in project management and leading change and transformation in Local Authority or similar settings. 

– Are good networkers, have experience of building strategic relationships and have a strong background working with and leading diverse stakeholders.

– Are excellent communicators, problem solvers and influencers.

You may also be responsible for line management of Project Officers and Management trainees. 

The 2 roles available are described below. Please state which role(s) you are interested in and in which order of preference when applying. 

1x Project Manager – Understanding Demand & Maximising Prevention – 2 Year FTC / Secondment

This role will join the team on a fixed term contract for 2 years and will be working on our prevention programme. This work covers a range of different initiatives that cut across the whole health and social care system; including changes to how we provide holistic support to people approaching social care for the first time, how we ensure people remain stable while waiting for an assessment or a review, and how we support those with the most complex needs to keep them well. As well as delivering partnership projects, the programme supports culture change within individual teams, ensuring all staff have the capability, motivation and opportunity to work preventatively on a day to day basis to prevent, reduce and delay support needs. 

2x Project Manager – Reablement – 1 Year FTC / Secondment

The role will join the team for 1 year on a fixed term contract specifically working  on the Reablement programme . Our vision is to take a “reablement and enablement first” approach to Adult Social Care and over the last few months we have piloted  this approach with residents being discharged from hospital . The next steps will be to expand this approach to include community based referrals which will require significant changes to current ways of working.
