Senior Social Work Practitioner  - Joint Emergency Team

£49,056 to £62,918 (Out of hours enhancements included)

This is an exciting opportunity for the right candidate who is looking for a new challenge by joining the Urgent Care Response Service in the Royal Borough of Greenwich called the Joint Emergency Team on a permanent basis as a Senior Social Work Practitioner.   

The JET team’ primary focus is to offer an agile and robust hospital admission avoidance service, focusing on managing health and social care crisis interventions in the community.   The team is staffed by both Oxleas Community Health Services and the Royal Borough of Greenwich Adult Social Care Service.  The team has GP’s, Nurses, Occupational Therapists, Physiotherapists, Social Workers, Occupational Therapists, Pharmacist and a Handy Person.  Due to the level of resourcing from across a raft of disciplines, JET is able to offer a comprehensive wraparound service to the people it works with.    You will have the opportunity to work flexibly and to collaborate with colleagues from different professional backgrounds who bring diverse expertise and a shared passion for helping people.

The role requires you to have excellent working knowledge of adult social care legislation and be able to implement principles of best practice. You are also expected to have some line management experience and be currently registered with Social Work England to join us. 

You will have some experience of supervising staff, be adept at participating in the day-to-day operations of workflow, can quality assure Care Act Assessments and perform as an Adult Safeguarding Manager.   You will be expected to carry a small caseload, and you may need to either be case accountable or co-work in very complex cases or situations when necessary and provide advice, support, and guidance to staff who are dealing with complex situations.  

This role does have an out-of-hours component, and the successful candidate will need to work one in four weekends to lead the social work staff in managing the discharge to assess pathway and community referrals.  You will be required to work either one or two lates per week (late shift: 12:30 noon to 8:30pm).   In addition, you will participate in the Out of Hours on call social worker service that runs from 8:30pm to 8am, Friday to Friday once a month.
