Advanced Practitioner Adults with Learning Disabilities

About the role


As an Advanced Practitioner, you will provide practice leadership within the Adults with Learning Disabilities team, supporting the delivery of high-quality, strengths-based interventions.

You will support the Team Manager and Assistant Team Manager in driving excellent standards of practice, while holding a reduced but complex caseload, including individuals with multiple and high-level needs.

You will also supervise a small number of staff and have strong strength-based practice with the ability to provide professional challenge and support.


Key Responsibilities

  • Provide expert practice leadership, mentoring, and reflective support to social workers and practitioners
  • Lead on complex casework, including safeguarding, risk management, and multi-agency interventions
  • Champion strengths-based, person-centred practice in line with the Care Act 2014
  • Apply and support others in using the Mental Capacity Act (MCA) 2005 and Best Interest decision-making
  • Complete S16 Welfare Application's and Lead on S21A Application's.
  • Support quality assurance, audits, and continuous service improvement
  • Work closely with health partners (including CLDTs who we are co-located with), providers, and community services
  • Promote independence, choice, and control, supporting people to achieve meaningful outcomes
  • Contribute to service development and innovation within learning disability provision

This is a key opportunity to shape practice and ensure adults with learning disabilities in Bury receive high-quality, person-centred support that promotes dignity and independence.

  • Passionate about improving outcomes for Adult with a Learning disability and their families
