We are inviting applications for the Social Worker in the Community Intervention Hub role.

Looking for your next exciting adventure in Adult Social Care? The London Borough of Bromley are looking for an experienced Social Worker to join our friendly and hard-working team as part of the new ASC front door model.  Working in conjunction with the Initial Contact Team, Safeguarding Team and based in the Community Intervention & Prevention Hub.

About the Role:

The role is in the front door offering focused short-term intervention to mitigate immediate risk and ensure that residents ‘wait well’ when further complex care management has been identified.  You will work alongside OTs in this new MDT approach, responding daily to referrals via the hub which deal with a broad range of Adult Social Care queries and referrals.  There is focus on supporting the Integrated Care Network (ICN) working collaboratively with health partners to offer short term intervention in the community.

Adult Social Care is on a transformation journey, and we are seeking talented and experienced people to join us to support the changes that are taking place and lead the team forward.

We are looking for a suitably qualified, professionally registered social worker to join our team.

You must have the ability and experience to maintain healthy relationships with partner agencies, particularly health, voluntary sector, and commissioned services. The ideal candidate should be conversant with different IT systems and know how to record information contemporaneously on social care systems.

You will possess excellent communication, analytical and written skills and be a problem solver, as well as be effective at prioritising, meeting performance targets and managing systems.

As part of a supportive management team, you will work to the requirements of the Care Act and Mental Capacity Act along with best practice, regular supervision, training, career development and the Council’s Appraisal model. We are committed to improving the lives of the people of Bromley by keeping them safe and supporting them to live independently.

If this sounds like the role for you, please do contact Leah Miller (Leah.Miller@bromley.gov.uk) or Gerard Coleman (Gerard.Coleman@bromley.gov.uk) for an informal discussion about this role.
