An exciting opportunity has arisen to join the Early Years SEN Inclusion Team as a Portage Home Visitor. The role is to provide a home teaching service for a caseload of children with severe and complex additional needs and their families, using the Portage model where partnership with families and muti- agency working is central.  You will be joining a team of committed and supportive colleagues.  The post also involves giving advice and support to mainstream pre-schools about strategies to support children as they move into a pre-school as well as doing groups for children and families. The post involves lone working.  We are seeking to appoint an enthusiastic and positive professional who will be a champion for children with SEND.

Applications will be assessed based on the criteria set out in the Job Description and Person Specification, which can be found here
