We are seeking a Care Assessor to join Brent’s Reablement Team—an integrated service that delivers fast‑paced responses to Home First discharges, alongside reablement and social care support through a short‑term intervention model. Our team ethos centres on promoting and enabling independence, and we maintain strong working relationships with partners and colleagues across hubs and intermediate care services.
This is an excellent opportunity for an experienced Care Assessor with a background in supporting adults with physical disabilities and older people. The successful candidate will work directly with Brent residents in their homes, carrying out Care Act Assessments and delivering reablement interventions.

The ideal candidate will hold a relevant professional qualification and have experience in Social Care and/or Intermediate Care, including working with Home Care and/or Reablement providers.
We are seeking a Care Assessor with strong communication skills and experience in a community‑focused, multidisciplinary setting. You will be passionate about supporting vulnerable adults and committed to delivering high‑quality services. Essential skills include multitasking, prioritisation, caseload management, and effective time management.
This role offers an excellent opportunity to work within a progressive, multidisciplinary team comprising Occupational Therapists, Social Workers, Care Assessors, and OT Assistants, all dedicated to providing reablement support to clients in their own homes.
