Job Introduction
We have exciting opportunities available within our Community Mental Health Teams, including roles in City CMHT, and CMHT South and West. We are seeking compassionate and skilled Social Workers or Approved Mental Health Professionals (AMHPs) to join our multidisciplinary teams, supporting adults with a wide range of mental health needs within the community.
You will be joining us at an exciting time as we continue to strengthen and expand our social work presence across community-based mental health services. We welcome both experienced practitioners and those completing their ASYE who have a strong interest in relationship-based, strengths-focused work with individuals experiencing complex and enduring mental health challenges. Qualified AMHPs will receive an additional £5,000 market supplement, bringing the full salary range to £47,893 – £51,142.
Our Community Mental Health Teams work with individuals experiencing a range of mental health conditions, including severe and enduring mental illness, often alongside complex social circumstances such as housing instability, social isolation, and substance misuse. You will work collaboratively within an integrated system alongside Bradford District Care NHS Foundation Trust (BDCT) and a wide network of partner agencies to deliver person-centred support that enables individuals to live as independently as possible within their communities. As part of this integrated approach, you will be required to undertake elements of care coordination, contributing to integrated care planning and ensuring that individuals receive joined-up, effective support across health and social care services.
We are looking for practitioners with strong interpersonal skills who can build trusting relationships and promote autonomy for individuals who may at times struggle to engage with services. You will be confident in assessing and managing risk within complex situations, supporting positive risk enablement, and empowering individuals to achieve outcomes that enhance their independence and wellbeing.
In this role, your responsibilities will include undertaking Care Act assessments, identifying eligible needs, and developing and reviewing support plans that reflect a strengths-based approach. This will include contributing to and implementing Section 117 aftercare plans to support individuals following discharge from hospital and reduce the risk of readmission. You will recognise and respond to Section 42 safeguarding concerns, ensure carers receive appropriate assessment and support, and contribute to statutory duties under the Mental Health Act and Mental Capacity Act.
