Job Introduction
We are seeking compassionate and skilled Social Workers or Approved Mental Health Professionals (AMHPs) to join our Assertive Outreach Team. This is a specialist service working with people who are often difficult to engage due to the complexity of their mental health needs, including psychosis and dealing with complex social situations such as homelessness and substance misuse.
You will be joining us at an exciting time as we strengthen and expand our social work presence within community‑based mental health outreach. We welcome both experienced practitioners and those completing their ASYE who have a strong interest in assertive, relationship‑based work with people facing multiple disadvantages. Qualified AMHPs will receive an additional £5,000 market supplement, bringing the full salary range to £47,893 – £51,142.
Our Integrated Outreach Team works with individuals who may not engage with traditional services and who require assertive, flexible, and creative approaches to support them in the community. You will work closely with Bradford District Care NHS Foundation Trust and a wide network of partners to deliver support that helps people remain within their communities.
We are looking for practitioners with excellent interpersonal skills who can form meaningful relationships with people who may struggle to engage with services. You will be confident in assessing and managing risk in dynamic situations, promoting safe and positive risk‑enablement, and supporting individuals to maintain independence wherever possible.
In this role, your responsibilities will include undertaking Care Act assessments, identifying eligible needs, recognising and responding to Section 42 safeguarding concerns, ensuring carers receive appropriate assessment and support, and contributing to statutory duties under the Mental Health Act and Mental Capacity Act. You will work with people whose needs span mental health, substance use, housing, physical health, and social care, applying person‑centred and strengths‑based approaches that recognise the impact of trauma, poverty, discrimination, and social exclusion. You will be expected to work assertively and persistently to engage individuals who may be reluctant to work with services, using creative and trauma‑informed methods to build trust, reduce risk, and promote stability within the community.
