The Role
We welcome applications from candidates with experience of:
Managing an active and varied caseload.
Undertaking safeguarding investigations.
Completing Continuing Health Care (CHC) assessments.
Taking part in multiagency meetings.
Working within a multidisciplinary team.
An exciting opportunity has arisen for a passionate and dedicated Social Worker to join Bolton’s Central North neighbourhood team, supporting adults and older adults within our local community.

What You’ll Be Doing
Carrying out Care Act 2014 assessments, including carers assessments.
Developing assetbased, strengthsfocused support plans that help people connect with universal, voluntary, and community services.
Applying the principles of the Mental Capacity Act 2005 and other key legislation to everyday practice.
Working collaboratively with partners across health and social care to support positive outcomes.

About You

We are looking to recruit an enthusiastic and motivated individual who has a passion for Social Work practice and has experience of working with adults in a community setting. You will have the ability to develop, maintain and improve relationships with service users and carers as well as the attributes required to participate in inter-disciplinary team working.

We welcome applications from Student Social Workers who are in their final placement and are due to qualify in the next 6 months.
