A key element of this role is to strengthen collaborative working, promoting effective multi-disciplinary team engagement within a fast-paced environment.
As part of the Discharge to Assess Social Work Teams, you will deliver person-centred practice, enabling individuals to maintain choice, control, independence, and resilience. Strong communication and interpersonal skills are essential, alongside the ability to produce clear reports and manage competing demands within tight timescales.
