Team Manager – Transition Team 

Permanent 

Grade 6 - £54,495- £64,811

Consultation grade - subject to formal evaluation under the Pay Equity Review

Working 36.5 Hours per week

We are seeking an experienced and motivated Team Manager to lead one of our Transition Teams, supporting young people with disabilities and additional needs as they move from Children’s Services into Adult Services.

This is an exciting opportunity to join an evolving service. The Transition Team is currently operating on a city‑wide basis and will transition to a North and South‑based model, aligned with the future structure of Younger Adults provision. The postholder will play a key role in leading through change and shaping future ways of working.

As Team Manager, you will provide strong leadership to a team of social workers and practitioners, ensuring high‑quality, strengths‑based transition planning in line with relevant legislation and statutory guidance. You will maintain oversight of performance, quality, risk and safeguarding, and work closely with partners across Children’s Services, Adult Social Care, Health, Education, providers and families to ensure smooth and person‑centred transition pathways.

About you
You will be a qualified social worker with current Social Work England registration and substantial management and supervisory experience. You will have a strong understanding of transition pathways, learning disabilities, autism and complex needs, and be confident leading teams through service change.
