An opportunity has arisen to join the Citywide Adults Impact Team (CWAIT). This role is subject to hybrid working arrangements, with a non‑negotiable requirement to be office‑based for up to two days per week. The postholder will manage complex caseloads and support citizens and their carers as part of a citywide service.
You must demonstrate your ability to remain resilient throughout the developmental changes CWAIT will be expected to accommodate that aligns with the Directorate’s vision for Adult Social Care.
CWAIT is seeking two experienced Social Workers to review and reassess high‑cost care packages, applying a strength‑based approach to maximise citizens’ independence while considering the effective use of Adult Social Care resources.
Successful candidates will be highly experienced practitioners with the ability to coach and mentor Band C Social Workers alongside their Senior Practitioner/Team Manager. You will be resilient, confident in working independently, and effective within a multidisciplinary environment, with a proven ability to support the development of others in a fast‑paced setting.
You must have experience of contributing to the development and promotion of service delivery, to enhance choice and control of person-centred support for citizens within their support planning process.
As part of your case management responsibilities, you will complete Care Act assessments, Continuing Healthcare (CHC) disputes, and comprehensive assessments and reviews to a high standard, ensuring strengths evidence‑based practice informs decisions and meets eligible care needs. You will be supporting vulnerable younger adults as part of your caseload responsibility, and this will often require confident approach whilst working with our legal services.
You will demonstrate sound professional judgement, defensible decision‑making, and professional curiosity when supporting vulnerable adults, particularly those younger adults with high level complex needs. You must be able to apply the principles of the 3‑Conversation Model and the Care Act 2014 to deliver person‑centred practice that promotes choice, control, independence, wellbeing, prevention, and early intervention. Strong partnership working skills are essential, alongside a clear understanding of the role and responsibilities of a Social Worker. You will also be expected to use innovative approaches to resolve challenges within your caseload.
Applicants must demonstrate experience of CHC processes and appeals, Section 117 aftercare planning and reviews, and the ability to manage disputes professionally, supporting colleagues where required.
