An exciting opportunity has arisen to join the Transitions Team. The team currently is subject of hybrid working arrangements, with an expectation of being office based (location will be determined by the Team Manager) 2 days per week, this being an essential requirement. Successful candidates will work with caseloads and citizens as part of its citywide service.
The Transitions supports 17-25 year olds, who has a diagnosis of Learning Disability, Physical Disability and Autism as they transition to adulthood. We are a unique service working across Birmingham City Council and Birmingham Children’s Trust offering statutory support for young people leaving the educational or care system with care and support needs. We are looking for innovative colleagues, with the ability to think outside the box, who are passionate about young people and put them at the heart of everything they do. This exciting opportunity has arisen for Social Work practitioners post grade 4 to join the Transitions Service. We are looking for an organised and dependable team player who has a positive and flexible approach with excellent communication and interpersonal skills, along with being enthusiastic and self-motivated.
You will be expected to support the Transitions Service whole system priorities using strength-based system approaches

You must be able to apply the principles and process which accompany the 3-Conversation Model and the Care Act 2014, to deliver on person centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on wellbeing, prevention, and early intervention to support.
You must be able to demonstrate partnership working with other professionals both internally and externally. You will be able to demonstrate your excellent application of the roles and responsibility of a social worker. Within your role, you be expected to use innovation to resolve challenges within your caseloads that you will be responsible for.
You will be able to demonstrate your knowledge and experience of the Continuing Health Care process / appeals and you will be skilled in Section 117 aftercare planning and reviews. You will demonstrate your ability and experience in working through disputes on a professional basis with other disciplines and you will support other team colleagues in disputes where required.
