Assessment & Enablement Officer

An exciting opportunity to be part of our small but effective Waiting Well Team, where you will provide targeted, preventative support for adults with social care needs and their informal carers.
This temporary resource is intended to support earlier intervention and contribute towards prevention focused conversations, practical support, and helping residents navigate appropriate services and community support where suitable.
You will work proactively with residents on localities waiting lists, recognising that support needs are likely to vary depending on individual circumstances. Using your skills and experience you will work alongside some residents who may require brief check-in conversations, reassurance, or information sharing, whilst others may require more regular follow up to help prevent escalation of need. The role will contribute towards operational performance indicators with specific measures and indicators to be confirmed as the work develops, whilst allowing flexibility to respond proportionately to differing levels of resident need.
Alongside direct resident contact, the postholders will also be expected to identify, log and share recurring themes, unmet needs and emerging pressures identified through waiting list conversations. Insights gathered through this work will help support wider service transformation, strengthen prevention opportunities and contribute towards a more joined up and responsive approach across Waiting Well priority areas and the wider system.
You will need the following skills and experience and in return we will provide you with excellent supervision, support and the tools to carry out your job effectively.
- a sound understanding of Health and Social Care eligibility
- experience of working with adults
- excellent interpersonal skills
- ability to use IT applications including Mosaic and Microsoft Teams
- knowledge of relevant social care legislation and guidance
- an understanding of protecting and making vulnerable adults safe
- ability to communicate with residents regarding complex issues
