We are seeking to appoint to the role of Adult Broker. Our Brokerage Service supports Adults’ Care and Support by brokering placements and packages of care. This includes residential & nursing care, home care, supported living, extra care and floating support. The Team is responsible for: • Brokering care packages and placements, ensuring quality services are purchased while ensuring value for money for the Local Authority. • Working closely with Social Workers to ensure the correct support is brokered. • Ensuring care is purchased using the Local Authority’s contracts and frameworks. • Working with social workers and representatives from the hospital to ensure timely discharges. • Ensuring the required contractual documents are in place The Broker role works closely with relevant operational colleagues and Commissioners to ensure Brokering tasks are completed in a timely manner.
