About this job

This is an exciting and unique role, working with children and young people who are looked after by the Local Authority or approaching leaving care, where there is no plan for them to live within their family or be adopted. The aim of Lifelong Links is to create a positive, lasting support network for children and young people, which promotes their identity and sense of belonging, therefore having a positive impact on their emotional wellbeing, reducing risk taking behaviours and promoting stability within their place of care.

Using the tools and techniques of the Lifelong Links model of practice, you will undertake specific direct work activities with the young person, explore and search for family members and other identified important people and engage their supportive adults. The Lifelong Links coordinator will facilitate Lifelong Links Celebration events (Family Group conferences) to support the creation of a Lifelong Links plan.
